Insurance
& Payment Arrangements
Dr. Karen Thomas is a PPO (not an HMO) provider.
As a PPO provider, Dr. Thomas participates as an in-network
provider in some plans (meaning that only a prescribed co-payment
is made by the patient). In other plans she is an out-of-network
or non-participating provider (meaning that a prescribed
annual pre-payment must be met after which 80% is covered
and 20% is paid by the patient).
PPO Insurance Plans in which Dr.
Thomas is an in-network provider and for which only a co-payment
is made by the patient:
- GHI
- Medicare
- Blue Cross/Blue Shield
- Oxford
- Some Union Plans
PPO Insurance Plans in which Dr. Thomas is an
out-of-network provider
and for which a deductible must first be met by the
patient:
- Aetna
- Cigna
- Guardian
- Prudential
- United Health Care
- Some Union Plans
HMO Insurance Plans in which Dr. Thomas does not participate:
Flexible Spending Plans are
another form of coverage that allows patients to be reimbursed
for treatments and services not covered by most insurance
companies. A flexible plan will reimburse for acupuncture,
nutritional supplements, orthotics, and other health aids.
Dr. Thomas will assist in filing the necessary paper work
to ensure reimbursement in such plans. Prior notice is requested.
Uninsured patients will find a sliding
scale is available for those with financial need.
Dr. Thomas makes every attempt to make all necessary treatments
available and affordable to all patients. Most insurance
does not cover acupuncture and reduced fees are available
in conjunction with other services.
Payment for services is due when
treatment is rendered and the following forms of payment
are accepted:
- Cash (of course)
- Checks (though only with established patients)
- Credit Cards (all major cards including: Am Ex, MC
&Visa)
